Olton ISD offers you a comprehensive benefits package to achieve this goal. You have several options to enhance your understanding and evaluation of this package.
IMPORTANT: Effective beginning on January 1, 2011, over-the-counter medicines or drugs (e.g. Advil, Ibuprofen, cough syrup) are not eligible for reimbursement under an FSA, without a doctor’s prescription. Insulin is the only medicine that doesn’t require a prescription. Also supplies that you need for medical care (e.g. contact lens solutions, bandages for wounds, thermometers) will continue to be eligible for reimbursement.
Important Things to Know for 2011 – 2012
- Olton ISD Cafeteria plan year is September 1, 2011 through
August 31, 2012.
- Medical: Olton ISD offers its employees the options
of ActiveCare I, II, III, or FirstCare for Medical coverage.
- Hospital Indemnity Plan: American Public Life will continue
providing the Hospital Indemnity Plan. Olton ISD will pay for the employee only
premium if employee waives Medical Coverage. For details, see brochure by clicking on Benefit Highlights/Hospital Indemnity Plan.
- Dental: Guardian will continue providing the Dental Plan.
Benefits are enhanced and rates are reduced from prior carrier. For details
on these exciting changes click on Benefit Highlights/Dental.
- Vision: Block will continue providing the vision plan. For details on these changes click on the Benefit Highlights/Vision.
- Disability: Unum Disability will continue providing the Educator Select Income Protection Plan. New coverage or increases in coverage are subject to pre-existing condition limitations. To view plan summary and rates, click on Benefits and Forms/Disability. Employees can enroll on a guarantee issue basis (no health questions asked).
- Accident: American Public Life will continue providing you with 24 hour Accident coverage for Emergency Room, Doctor, Ambulance, Hospital and Accidental death.
- Voluntary Life: Dearborn National will continue providing the Voluntary Life. Existing employees that have enrolled in Voluntary Life for themselves or their spouse may increase their coverage by $10,000, up to the Guarantee Issue amount, without Evidence of Insurability. Employees who declined Voluntary Life coverage during the initial eligibility period and elect coverage must submit Evidence of Insurability application.
- Permanent Life: Texas Life will continue as the Individual Permanent Life provider. Coverage can be obtained on Employee or Spouse or Children and Grandchildren. The Employee does not have to have this coverage to cover family members.
- Flexible Spending Accounts: Mass Group Marketing will continue as the Third Party Administrator for the Un-reimbursed Medical Account and the Dependent Care Account. Employees may shelter up to $3,000.00 annually in the Un-reimbursed Medical Account and up to $5,000.00 in the Dependent Care Account.
